Tryout Registration Form - 2020 Season

Any player who plans to tryout for the team in February 2021 (for the 2020 season) needs to complete this form.

IN ADDITION:
1. Each player who plays on a club/travel sports team needs to email her practice schedule and tournament schedule covering the period of February 15th through April 25th to Coach Magula (emagula@verizon.net). If you do not play on a club/travel sports team, please email this information to Coach Magula.

2. In the email for item 1 above, please include the following statement and your answer:
One thing I wish the coaches knew about me that would help them coach me better is….

Please remember that a current physical, dated on or after May 1, 2020, is required for participation. Completed and signed physical forms should be submitted to Mr. Adam Mistr, the Athletic Trainer at FC.

OH = Outside Hitter; MH = Middle Hitter; RS = Right Side Hitter; S = Setter; DS = Defensive Specialist (play back row only); L = Libero (a specialized back row player). A Serving Specialist is a player who serves and plays back row defense until her service rotation ends.
